Organizational Information

  • Name of organization
  • Organization Executive Director/CEO
  • Month and year organization was incorporated?
  • Brief history of your organization and your vision for the future
  • Describe your programs/services and the beneficiaries of those services.
  • How many individuals does your organization serve in Winnebago Counties?
  • What outcomes (changed behaviors/conditions) are resulting from your programs/services?
  • Describe major affiliations and partnerships with other entities (both nonprofit and/or for-profit)

Endowment Vision (please be concise)

  • Explain why an endowment fund is important to your organization.
  • How might this endowment gift impact your mission work?
  • Explain your organization’s strategy for using and growing the endowment.
  • If applicable, provide a brief history of your organization’s current endowment.
  • Describe how your endowment will leverage other opportunities or assist in meeting defined goals. (attach your goals/strategic plan/long-term strategy)
  • Will your application support a collaboration? If so, please list your formal collaborative partners and share how an endowment will support expanded collaboration and impact within your ecosystem.

 

Attachments

  • Provide a letter signed by the organization’s Executive Director/President/CEO and the Board President/Chair, that:
    • States the organization’s commitment to apply.
    • States the organization’s KFF endowment fund request amount not to exceed $1M.
    • Verifies that the information stated on the organization’s application is accurate and complete to the best of both parties’ knowledge.
  • By Laws
  • Most recent audited financial statements and 990.
  • Provide a list of board members and their community and professional affiliations.
  • If applicable, a complete copy of the most recent accreditation or licensure report.
  • IRS ruling letters indicating that the organization is a 501(c)(3) tax-exempt organization.

  • Certificate of Good Standing from the State of Illinois. Business Search / Certificate of Good Standing
